预激综合征（Pre-excitation syndrome）或WPW（Wolf-Parkinson-White）综合征，是一种异常房室传导途径为病理基础,以异常心电生理表现和/或易并发多种快速性心律失常为特征的临床综合征 。

【简介】

预激的病因是正常房室传导系统以外的先天性房室附加通道(简称旁路)存在,冲动经附加通道下传，提前兴奋心室的一部分或全部，引起部分心室肌提前激动。大多无器质性心脏病。旁路有以下几种：1.房室旁道(Kent束)。最常见,大多位于左、右两侧房室沟或间隔旁，连接心房肌和心室肌；2.房结旁道(James束)。为心房与房室结下部或房室束的通道，可能为后结间束部分纤维所形成；3.结室、束室连接(Mahaim纤维)。为连接房室结远端或房室束或束支近端与室间隔的通路。

房室折返性心动过速(Atrioventricular reciprocating tachycardia, AVRT)是由旁路参与房室之间的大折返性心动过速。

【简介】

AVRT 的典型旁路是房室结外连接心房和心室肌的通道,其分类是基于沿二尖瓣或三尖瓣的部位不同而定,电生理特性为快的、非递减传导,类似正常希-浦系组织和心房或心室肌的传导。旁路具有前向(显性旁路)或逆向传导功能(隐匿性旁路)或兼而有之,约8 %的旁路显示递减的前向或逆向传导。AVRT 以房室结的传导方向分为前向和逆向AVRT,前者折返激动的传导是经房室结前传心室,经旁路逆传心房;后者前传经旁路到心室,逆向是经房室结或第二条旁路到心房。
